The business four-ball includes a sponsor sign on a tee box. Tell us what it should say and upload your logo so the sign writer can make it.

Sign wording / company name * Upload your company logo for your tee sign — PNG, JPG, PDF, SVG, EPS or AI, max 20MB

Vector files (PDF, SVG, EPS, AI) print best. No logo to hand? You can send it later – we'll remind you.

Players (you can add or change names later – leave handicap blank to play off 18)

Player 1 under 18
Player 2 under 18
Player 3 under 18
Player 4 under 18

Names and scores appear on the public live leaderboard on the day. Under-18s are only ever shown as initials.

If any player is under 18: I confirm their parent or guardian agrees to them taking part and to the club holding their name and handicap for the event. Notes (optional) I've read the privacy notice and I've told the players named above that their details are shared with the club for the golf day. Optional: email me about future Sky Blues fundraising events. (You can unsubscribe at any time.)

Payment is made to the club treasurer, not online. Sending this form is a request – your time is only booked once the committee confirms it.
